Steve didn't know what to expect when he heard about "Thanos" but he's seen aliens come down and attack New York, so it wasn't shocking. Steve had fought with every fiber of his being as these new aliens attacked him and everyone on Wakanda.

He had fought for what felt like hours, his muscles burning in exhaustion, his body screaming at him to stop doing this to himself. His mind would never let him. He continues to fight again and again until everyone could go home safe.

That's why when he finally saw Thanos walk out of that portal, his first instinct was to fight. Thanos could not get that stone.

He fought with every fiber of his being until one fatal blow knocked him unconscious and when he woke up, the first thing he sees is Bucky turning to dust before his eyes.
